- Amikacin sulfate indicated in the short-term treatment of serious infections due to susceptible organisms
- [With the exception of uncomplicated UTI, aminoglycosides are generally used in combination for serious infections including treatment of Pseudomonas aeruginosa.]
- Bacterial septicemia (including neonatal sepsis)
- Respiratory tract infections
- Bone and joint infections
- Central nervous system infections
- Skin and soft tissue infections (including burns, postoperative infections)
- Intra-abdominal infections
- Complicated and recurrent urinary tract infections.

- Pneumonia, hospital-acquired (in combination with a beta-lactam, beta-lactam/beta-lactamase inhibitor, or a third-/fourth-generation cephalosporin)
- Tuberculosis
- Non-tubercular mycobacterial (NTM) infections, MAI
- Nocardia
- Aerosolized amikacin has been used for the management of cystic fibrosis exacerbations
