  • Histoplasma capsulatum (most common)
  • Mycobacterium tuberculosis
  • Coccidioides immitis
  • Aspergillus
  • Actinomyces
  • Wucheria bancrofti
  • This may also be due to noninfectious etiologies.
    • Sarcoidosis
    • Rheumatoid arthritis
    • IgG-4 disease
    • ANCA-related vasculitis
    • Lymphoma-related
    • Radiation
    • Drug therapy (methysergide)
  • Idiopathic: if etiology is undetermined.
