Leading causes:

  • Staphylococcus aureus, including MRSA (accounts for ~70%)
  • Gram-negative bacilli (infrequent)
    • Pseudomonas aeruginosa most common GN in the U.S.[2]
  • Mixed infections with anaerobes
  • Streptococcus spp. including S. pneumoniae
  • Mycobacterium tuberculosis
  • Immunocompromised hosts (especially advanced AIDS): Aspergillus, mycobacteria (TB, MAI), Salmonella, and other Gram-negative bacilli
